2012-2013 Wayzata High School Athletic Registration Information
All participants in athletic programs at Wayzata High School must be fully registered prior to the athlete’s first practice/tryout.
Note: Student-Athletes may participate in only one athletic program per season.
A student-athlete who has transferred to WHS during their high school career, is moving into the school district, or is a foreign exchange student should register in person accompanied by a parent/guardian.
Refunds: If you register, but change your mind or don't tryout, your card will be refunded your payment minus a $10 administrative fee. If you tryout and do not make the team, your card will be fully refunded.

Monday, July 9 – Wednesday, August 24. You must complete the following steps to be cleared for participation:
Step 1:
Current Sports Qualifying Physical Examination dated after
6/15/10 -
must be on file in the WHS Activities Office
Step 2: Submit the completed
2012-2013 WHS/Minnesota State High School League Activities & Athletics Eligibility Form - must be turned in once per school year
Step 3: Fee payment
Note: You may receive an email if the steps are not completed in order.
PLEASE READ!! If you pay online without having these forms on file, you will not be considered registered and you will not be allowed to participate or receive any equipment until we have the forms. Physicals are not transferred from the middle school to the high school Activities Office.
Physicals must be signed and dated by a physician. We only need the one page Sports Qualifying Physical Examination Clearance Form.
